We offer quick and easy testing for ages 6-60 conducted by a clinician with over 16 years of experience diagnosing ADHD.  We use a combined method of a clinical evaluation and an FDA-cleared medical device (QbCheck) that helps with the diagnosing of ADHD.

COMPLETE REQUIRED DOCUMENTS


WHEN OUR TEAM MEMBER REACHES OUT TO YOU, YOU WILL RECEIVE ALL NECESSARY DOCUMENTS TO FILL OUT PRIOR TO YOUR VISIT. AT THIS TIME,  A $150 NON-REFUNDABLE DEPOSIT WILL BE REQUIRED. ONCE THE DEPOSIT IS RECEIVED, YOU WILL BE ABLE TO SCHEDULE AN APPOINTMENT. PLEASE NOTE, ALL DOCUMENTS MUST BE COMPLETED IN THE PORTAL PRIOR TO THE VISIT IN ORDER TO GIVE A PROPER ASSESMENT.

EVALUATION & TESTING


APPOINTMENTS CAN BE DONE IN-PERSON OR THROUGH OUR TELEHEALTH PORTAL.

ONCE YOU HAVE BEEN EVALUATED AND OUR CLINICIAN HAS DETERMINED YOU ARE A CANDIDATE FOR TESTING, THE REMAINING BALANCE OF $200 WILL BE REQUIRED TO CONTINUE. ONCE COMPLETED YOU WILL BE SENT TO OUR QbCheck STATION.  IF YOU ARE VISITING VIA AN ONLINE APPOINTMENT, YOU WILL BE SENT A LINK TO COMPLETE THIS PROCESS AT HOME.

RESULTS


AFTER COMPLETION OF QbCheck AND EVALUATION, YOUR CLINICIAN WILL UPLOAD YOUR REPORT AND EVALUATION INTO THE PORTAL FOR YOU TO ACCESS ANYTIME YOU NEED IT. THERE IS AN OPTION TO RECEIVE A POST VISIT CALL TO GO OVER ANY RESULTS AND EVALUATIONS.  WE WILL SEND THE PAPERWORK TO A PROVIDER UPON REQUEST AND COMPLETION OF A RELEASE OF INFORMATION DOCUMENT.

Who can be tested?

Our team provides comprehensive ADHD testing accessible across all of Florida for ages 6 to 60!

What is the Qb Check Test?

An FDA cleared assessment that objectively measures ADHD core symptoms using infrared technology, taken on the computer.

How long does the test take?

The test takes approximately 20-30 minutes in person at the office. Please plan for a 45-60 minute appointment.

What are the fees and services included?

$356 total (we cannot take insurance for this service). This amount includes the initial paperwork, rating scales, clinical interview, Qb Check test, report fee, explanation of results, and sending of reports to medical providers upon completion of a release form. There is a $3 processing fee per card swipe (included in cost above)

Good Faith Estimate


Healthcare providers must provide a Good Faith Estimate in writing for non-emergency services and items at least one business day before the service. Patients can also request an estimate before scheduling a service. If a bill is at least $400 more than the estimate, patients can dispute it. (Our price is $356 total including a $150 dollar non refundable initial evaluation visit deposit, $200 upon meeting the criteria to continue, and 2, $3 dollar processing fees for swiping your card.)


Under Section 2799B-6 of the Public Health Services Act, health care providers and health care facilities are required to inform individuals who are not enrolled in a plan or coverage or a Federal health care program, or not seeing to file a claim with their plan or coverage both orally and in writing of their ability, upon request or at the time of scheduling health care items and services, to receive a "Good Faith Estimate" of expected charges.

You have the right to receive a "Good Faith Estimate" explaining how much your medical care will cost.

Under the law, health care providers need to give patients who don't have insurance or who are not using insurance an estimate of the bill of medical items and services.

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees,

  • Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you chose, for a Good Faith Estimate before you schedule an item or service.

  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
